3. For minor's, write the minor's name and signature

MEDICAL RELEASE INSTRUCTIONS

1. Download and print MEDICAL RELEASE

2. Write your name, date of birth, and social security number at the top

3. Signature and today's date at the bottom, leave everything else blank

4. Print one medical release for everyone in the accident

5. For minor's, follow steps 1-4 and signature above authorized representative
